In this special Sunday Spotlight episode of Uncomfortably, Beautifully Human, we're shining a light on a remarkable organisation: Reach Out Hertfordshire.

About Reach Out Hertfordshire:Reach Out Hertfordshire is a home from hospital volunteering project. Our volunteers support older people who have recently been discharged from hospital, or are medically vulnerable, and are at risk of admission to hospital. We have been operating in Herts for over 4 years and work with local hospitals, Herts Help, Social workers, GP social prescribers and other charities to support people and help them during their recovery. Your commitment is an hour a week to visit a client, this could entail a cup of tea and chat/ collecting light shopping and prescriptions / going for short walks/ building confidence and independence/ and much more.

Join me in a heartfelt conversation with Nikki Lane from Reach Out Hertfordshire as we discuss:

✅ How Reach Out is bridging the gap between hospital and home✅ The vital role of volunteers in cultivating confidence, connection and companionship✅ Real stories of impact and the power of human connection✅ How YOU can get involved and make a difference

Why This Conversation Matters

Many older adults find themselves vulnerable and isolated post-hospitalization, and without the right support, their recovery can be challenging. Reach Out Hertfordshire is tackling this issue head-on, offering companionship, practical help, and encouragement—all through the dedication of volunteers who give just an hour a week.
